The Town has a Health Reimbursement Arrangement (HRA) for all health insurance plan subscribers. The Town will reimburse an employee or pay a medical facility directly for any actual out-of-pocket cost for the following covered services:
- Outpatient Surgery Co-payments (up to $250 per service after deductible)
- Inpatient Hospital Admission Co-payments (up to $500 per admission after deductible)
- High Tech Imaging Co-payments (up to $100 per service after deductible) e.g., MRI, CT and PET scans-please note that x-rays do not have a co-payment. Costs associated with an x-ray would be to fulfill your deductible.
- Plan year Deductible in excess of 50%.
If you have a medical claims bill/invoice for any of the above co-payments, you may submit for a reimbursement or direct payment through the Online HRA Claims Submittal. In order to complete this claims submittal, you will need to upload a copy of your medical claims bill/invoice. Please have it ready. Also, if seeking a reimbursement to you, the employee, you must upload proof of payment.
You may also submit a paper copy of the HRA Claim Form (PDF) to the Human Resources Department. However, the online application is strongly preferred.
